Mental Health Advance Directives

Originally Presented on March 13, 2014

Act 194 of 2004 allows you to create a Mental Health Advance Directive – which can include a Declaration and/or a Mental Health Power of Attorney. The law promotes planning ahead for the mental health services that an individual might want to receive during a crisis if unable to make decisions. A Mental Health Advance Directive is a tool that focuses on wellness and recovery planning. The webinar training will assist participants in understanding the law and how it works.

(The handouts listed below are the same slides, with two different options for how the slides are displayed)

2 Slides Per Page: (Please note, this will print two slides per page, therefore using a lot of paper and ink, but will be easier to read) 3 Slides Per Page: (These will print out three slides to a page, with lines for notes on the right of the page, but may be a bit more difficult to read due to smaller font.)
